Terms and conditions
Terms and conditions apply: Receive a comprehensive summer whitening campaign combination package at Forum Dentistry for $799 (Initial Consultation and Treatment). A consultation will be required before any treatment is performed. You will be required to fill out a medical history form when you attend the Forum Dentistry practice (and prior to your consultation) to assist our dental professionals' evaluation. All treatments are subject to our dental professionals' clinical assessment of suitability for you.
During the Initial Consultation and Treatment, our dental professionals will:
- discuss your whitening goals;
- provide information about whitening treatment options;
- evaluate your teeth condition to consider whether whitening treatment may be suitable for you;
- complete a teeth shade assessment;
- provide a professional clean;
- perform a Zoom! in-chair whitening session;
- provide a take-home whitening kit (custom dental trays, whitening gel kit); and
- provide a complimentary Philips Sonicare Electric Toothbrush.
The consultation does not include additional dental treatments that may be required before commencing whitening treatment. The $799 package price applies to the complete combination treatment only and cannot be redeemed as cash or applied to other dental services.
Offer Period
This Offer is valid for patients who make an appointment at Forum Dentistry during the period between 12 November 2025 to 28 February 2025, subject to appointment availability.
Redemption Process
To claim this Offer:
- Book an appointment at Forum Dentistry during the Offer Period
- Mention the Summer Whitening Campaign Combination Offer at the time of booking
- Attend your scheduled appointment
- Complete the Initial Consultation where suitability for treatment will be assessed
- If deemed suitable, complete the whitening treatment package
Offer Restrictions
Whitening treatments are not available for purchase by the general public. They will only be offered following the consultation process as set out in this Offer. The following restrictions apply to the eligibility and use of the Offer:
- you must be a new or existing patient of Forum Dentistry;
- only one Offer can be redeemed per person;·must be deemed suitable for treatment by our dental professionals following clinical assessment during the Initial Consultation;
- must be 18 years or older; and
- this Offer cannot be used in conjunction with any other offers.
